What is your motivation for applying?:
My motivation for applying to be an employee of the Karakura Hospital is to further develop my roleplay experience, while using what knowledge I have of the medical field. Roleplaying as an adult in Karakura, I may have many freedoms but that can tip to the boring side on occasion; which is why the position of Surgeon strikes me, as it is an occupation that requires deft attention.
What do you know about medical practice?:
I was in a Medical program for three years, I however did not follow that path as it just wasn’t my calling. I still retain much of the knowledge that was taught to me during this program; ranging from safety conduct and protocol, various hospital procedures, sterilization, and how to use many medical tools.
What are the basic color codes in a medical emergency and hospital?:
Code pink: Infant or child patient abduction.
Code Amber: Missing or abducted visitor child.
Code green: Emergency Operations activation.
Code orange: Hazardous material, or spill.
Code White: Evacuation.
Code brown: severe weather
Code red: fire emergency
Though some hospitals may vary, many use a coding system similar to the one listed above.
Do you acknowledge that if you are inactive you will be demoted?:
Yes, I acknowledge if I am given the privilege of roleplaying under this position, then being inactive will result in my loss of it.

Yes, it is more realistic that way as some positions require more education.

How is your character during on and off the job?
Michael always has a calm temperament, and is motivated to get done what needs to be done, on and off the job. Michael exercises daily and maintains his health on a keen basis.
What do they look like?
Michael A. Ishida stands 6’1, weighing 178 lbs and having a mesomorphic build. Michael’s body is in a fit physical condition. Michael has dirty blonde hair, and blue eyes.
Do they have any mental or physical illnesses?
Aside from his short-lived alcoholism, Michael has a healthy state of mind. Michael is physically healthy as well.
What makes your character unique?
Michael is an average looking person; however, his thought process is different from that of a regular person. Michael perceives everything in wholeness, while most people selectively and subconsciously perceive information.
BACKSTORY
Michael Adam Ishida would be born on August 3rd of 1989, in Osaka, Japan. Michael’s mother, Isabela Ishida, would die on December 12th of 1999, which is when his father, Hiro Ishida moved him and Michael to Florida, in the United States of America, at the age of 10. Michael’s English had been very weak upon arriving in the United States, with Michaels basic English, he would eventually speak English fluctuant and flawless by the age of thirteen. Michael’s path begins to slip up, as he associates with the wrong people and almost turns to a life of crime at a young age. Thanks to his Father’s guidance, he is left following in his footsteps to become a surgeon, like him. Michael would eventually be accepted into the University of Miami’s School of Medicine. Several months into his first undergraduate year, his father, Hiro, dies of brain cancer. Michael had become an alcoholic, drinking away his thoughts in utter despair. Michael would eventually overcome his alcoholism, seeing that it was hindering him from success in the academic world. He had almost lost everything, his grades, family, friends and home, but he hadn’t lost sight of one thing: aspiration. Aside his college hood financial troubles, he kept pushing through and making investments to better his living. Upon Michael’s graduation in 2014, he has been very determined to perform the best job wherever he works.
