Lost Item (Again)

Issue: My item disappeared in my offhand when I opened the school planner (It happened literally about 5 minutes ago with me making this post, this has happened once before with the same item, if the staff could be gracious enough and give me the item again I will try my best not to make the same mistake a third time)
