Additional Information:
This happened with the current bug where right clicking with an item in your off-hand swaps it in your current hand. If you do this with the school planner in hand, as this is an item that cannot be moved, your current off-hand item gets deleted.
If that can help in any way...

Steps to reproduce:
[Swapping]
- Have an item in off-hand
- Right click (checking someone's description, using your main hand item, opening a door, whatever)
[Deleting]
- Have an item in off-hand
- Have your School Planner in main hand
- Right click to open School Planner
