What is your discord name & tag?:
tzutbz

Previous bans:
I have never been banned.

Describe your activity on the server:
I just recently returned to SRP after taking a break from Minecraft as a whole. Whenever I open Minecraft now, I have the intention of joining SRP and nothing else. Due to this, I play for at least 3 hours a day and maintain constant activity. With summer approaching, I expect to have even more time with school being out of the picture.

What is your timezone?:
Pacific Time (PT)

List your current and past applications:
Due to the fact that I lost access to my old forum account from 2019 and before, I am unable to access past applications as they were from 2017-2018. I don’t believe I could even access the email used on the forum account. Currently, I have no roles besides college student on my alt (aespas). The roles I applied for and got accepted as back then included Teaching Assistant, Teacher, and Shop Owner. I believe I also had accepted language applications.
At the moment, these are the following applications I have on this account:

What experiences do you have with roleplay?:
I have branched out and explored a myriad of different roleplaying scenarios. Prior to my break, I remember being involved in the usual high school experience as well as serving as a teacher who planned out agendas with objectives for classrooms. I appreciate that the opportunities are endless when it comes to roleplay. It is because of this I say that I have only explored a minuscule part of roleplay as a whole. I have learned to craft stories for individual characters that are completely polar from one another, creating that sense of uniqueness that drives roleplay. In doing so, I also have experience with changing my approaches to situations. Whether it be a polite and extroverted person who is able to befriend anyone or a rather cold and introverted person who fears walking up to a cashier, I make sure that my responses are calculated. In addition to this, I have also experienced medieval settings where I had to completely change the process of crafting my characters and adapt to the circumstances of the particular time. I find these small adjustments to be very interesting as they test an individual’s ability to tell stories through their characters.

What is your motivation for applying?:
My motivation for applying centers around what the Karakura Hospital symbolizes and represents in SRP. Yes, concepts like CrimeRP are essential components to the complexity of SRP however, the hospital serves as a haven for citizens. I would love to experience the role of a physician who makes the health of everyone in town his top priority. As a doctor, you play a crucial part in ensuring that everyone in Karakura has a place they can look to if they ever face something that impacts their health. In a way, the clinic serves as a place of reassurance that people will be taken care of. The roleplay experiences that take place at the clinic are very unique and cannot happen anywhere else. For one, RecoveryRP has become a very popular concept in SRP. The most common times when this happens are after surgeries. I want to be able to experience RecoveryRP through the lens of a physician as patients heal. I find that this aspect of the clinic is quite inquisitive.
Furthermore, I hope to become part of the faction and take on an entirely different approach to roleplay. Given that I am not part of any factions at the moment, I want to branch out and roleplay different experiences with Karakura citizens that need medical assistance. Through these encounters, my actions and means of engagement with people will be precisely detailed.

Which role are you applying for?
(Surgeon, Doctor, or Psychiatrist):

I will be applying for the role of a doctor.

What knowledge do you have of Hospital Work?
Hospital work is very demanding and it requires a level of focus that keeps you agile. Due to the fact that patients will come in with different concerns, you must be ready to make yourself flexible and apply the techniques you have learned in training for different scenarios. Yes, there may be days when all the booked consultations are simple check-ups however, this will not always be the case. It is up to a doctor to be able to ****yze a patient’s symptoms correctly, diagnose them with the proper conditions, and prescribe the correct medicine. In addition, doctors must be able to ****yze a patient’s record history and also build conclusions based on their consultations. At times, a patient may not even know the cause of why they are feeling something. This is when doctors may perform diagnostic tests to view any underlying conditions that may be causing them harm. Furthermore, a doctor could be someone’s saving grace when they uncover something like a tumor that is in its earliest stage. Things like these can be uncovered through the usage of MRI scanning, where a physician can ****yze a person's internal tissue. This way, a patient can be saved before a concern becomes very severe. A doctor may also need to utilize technology like X-Rays to figure out what the best possible route for helping a patient’s injury may be. In addition, they may also take blood samples to ****yze body functions like the liver and kidneys. This will be especially important when someone takes a heavy medication that is being processed through a particular organ. This can prompt a doctor to change the amount they prescribe. An example of this is when a doctor adjusts the milligram amount of a medicine to accommodate a patient’s body. Emergencies will be a constant scenario that occurs in the hospital. Staff, particularly doctors, must be ready to salvage as much time as possible to save a patient. In most cases, the person will be going through a life-altering condition that physicians will need to act quickly on. However, there will be times when a doctor will not be able to treat what a patient is going through. This is when other medical staff must join in to find what is best for the patient’s health. A surgeon may need to be called in to perform surgery on a patient who has a condition that is far too advanced for a medication to cure.

On SRP, there are three major specialists at the hospital. First, the one I described in depth is the doctor. One of their peers are the psychiatrists, who handle the mental health aspect of patients. You can find them anywhere in the clinic, although the most probable area would be the fourth floor which houses the therapy ward. They are responsible for listening to patients alongside ****yzing their behaviors. As a result of doing this, they have the responsibility of providing treatments that can range from prescribed medications to the therapy itself. Another peer of doctors are the surgeons. You can also find these physicians anywhere in the clinic, however, the most probable area would be the second floor which houses emergency and operating rooms. They are responsible for the intense medical practices we know as surgeries. At times, doctors will not be able to properly treat a patient without them undergoing a particular surgery. An example of one of these many surgeries could be appendicitis surgery where the surgeon must make a large cut on the patient’s abdomen to halt the excruciating pain. Each group of specialists also has clinical leads who oversee the duties of each staff. Finally, there is also the hospital director who facilitates many responsibilities at the clinic. Essentially, the director manages and oversees every group of medical staff to make sure everyone is doing their job accordingly. As a team, this group of physicians works hand-in-hand to provide the best care possible for Karakura patients. Furthermore, you may also find these figures behind the desk of the pharmacy where you can pick up your prescriptions and begin your healing journey.

Do you acknowledge that you’re subject to being demoted at any given time if accepted?
Yes, I acknowledge.

Do you acknowledge that by applying for this role, you agree to be 100% dedicated to the Hospital Staff role?
Yes, I acknowledge.

Do you acknowledge that if training is held whilst you are online, you are expected to attend or you will be punished?
Yes, I acknowledge.

Do you acknowledge that you are NOT to take things OOCly when dealing with training & situations?
Yes, I acknowledge.

Backstory (100+ Words)
Henry Mateo was born on January 10, 1989, in Albuquerque, New Mexico in the United States of America to a Mexican father and a Japanese mother. They grew up in a rather poor neighborhood and sometimes, they didn’t have enough money to put food on the table. By the age of 4, his parents were divorced and they shared custody of Henry. This went on until Henry reached middle school when he began having issues with his identity and started questioning his sexuality. His mother constantly supported him through everything however, his father was not supportive in the slightest and would constantly repeat statements of disapproval toward Henry being gay. This drove a wedge in the relationship Henry once had with his father. Middle school only got worse when he transitioned into adolescence and experienced severe acne. At the time, most of his peers had not encountered the severe breakouts that followed him for years. Not only did he feel self-conscious, but he also felt like he wasn’t worthy of friends because of his appearance.
Henry moved to Fukuoka, Japan, for his final year of middle school and he was living with his mother in a small apartment. There was essentially no contact between Henry and his father anymore. Although Henry felt very hurt during the first 2 years of living with his mom, he soon felt a sense of relief not having to be burdened by the disapproval of his father. In addition, his mother now had a stable job that could afford a consultation with a dermatologist for Henry. He was prescribed acne medication that cleared his skin and boosted his self-confidence immensely. During 9th grade, he found a group of friends that were not only supportive of him but also identified with the same community as him. This encouraged Henry to have a different, more positive outlook on social interactions compared to his perspective in middle school.
In high school, his favorite subject above all was science. Whether it was a Chemistry class or a Biology class, he found a way to make connections between the lessons he learned. In discussions and labs, he also flourished as he helped people who didn’t understand certain topics. He excelled in school and with a healthy environment at home, he was able to prioritize his studies and get accepted into his dream college. He spent his summers there volunteering at a myriad of labs and hospitals, where he fell in love with the idea of being a doctor. Throughout his volunteering experiences, his favorite part was hearing the stories of patients who came from different circumstances. He was happy to help them in any way that he could. By the time he applied for medical school, he knew exactly what he wanted to specialize in as well.

SECTION 2: MEDICAL HISTORY
What is your character’s medical specialty? Why did they choose this focus?
(Ensure this is appropriate to the position you have chosen)

Henry’s medical specialty is Dermatology. More than anything, he resonates with people who feel self-conscious about issues like their skin, scalp, hair, etc. Growing up, he experienced unfair treatment from peers because he had acne. It took years before he booked a consultation with a dermatologist, who not only made him feel heard and much better about his skin concerns, but also provided him with a medication that changed his life. Having been prescribed Isotretinoin, his acne disappeared and he wished that he visited a dermatologist earlier on in life.

What medical residencies did your character take after medical school?
Henry completed a 3-year medical residency program in the Department of Dermatology.

Does your character have any work experience in medical positions?
Shortly after obtaining his Bachelor of Science in Biology, Henry worked as a Research Lab Assistant at a medical school over the summer. Through his experience there, he learned about several molecular biology techniques that involved DNA and bacteria. During his singular gap year between undergraduate and graduate school, he also worked in a dermatology clinical fellowship where he learned about skin disease screenings. After completing his residency program, he also worked as a dermatologist located in Hiroshima, Japan.

Has your character completed any additional schooling?
At the age of 32, he decided to go back to school for a Master's in Healthcare Quality and Safety Management. He was interested in pursuing the degree because it expanded his knowledge even more when it came to patient safety, which was his priority. The program taught him what it meant to perfect medical procedures and lead safety initiatives to make patients feel more secure.

SECTION 3: CHARACTER KNOWLEDGE
Describe your character; how do they look, what makes them unique?

Henry is 72 inches tall and weighs 153 pounds, creating a rather slim figure. He has black, wavy hair with hazel eyes. He lacks any facial hair, resembling a cleanly shaved face. His skin is rather pale and he has a ‘dewy' look, probably due to the sunscreen he reapplies every few hours. He also has a light touch of pink blush on his cheeks. He consistently smiles at the people around him because he likes living by the saying, “A smile can mean a thousand words”. He can be described as someone who is constantly complimenting people and likes starting new friendships. He enjoys socializing however, when it comes to completing work, he makes it his top priority and does not like any disruptions.
He is never rude, regardless of the situation he finds himself in. He is able to keep his composure in stressful situations as well, something he learned to do in medical school while balancing several responsibilities. He appreciates the conversations he is able to have with patients, as he hears their individual stories and the different circumstances that each one comes from. He is particularly touched by the stories regarding skin concerns like acne. He resonates with their stories since he recalls how he used to feel before he visited a doctor. The first thing he always does during consultations is try his best to make the patient feel heard.

How does your character act on and off duty?
On duty, Henry has a serious demeanor while still showing a warm smile to the people around him at the clinic. He is heavily focused on his work and his main priority is ensuring that every patient leaves feeling better than they did before. During consultations, he makes sure the patient feels safe and heard. He constantly refers to the concept of the patient-physician relationship that he learned about when studying for his Master’s degree. He acknowledges that patients should be able to build trust in their doctors. He does everything he can to make sure that patients know they are in good hands at Karakura Hospital.
Off duty, Henry continues to spread his smile to the people he passes on the sidewalk. He realizes that some people are going through their own personal struggles and a smile is able to make someone’s day. Although he is a very social person, he doesn’t value things like partying and clubbing as most people would expect. He loves activities like discussing a book he has read and also painting.

Does your character function better on their own or with others?
Henry never shies away from the opportunity to collaborate with the people around him. He believes that everyone has unique perspectives that can add a lot to the main objective. In this case, that is making sure that every citizen in Karakura gets the utmost care in the clinic. He acknowledges that not every physician is suited to take care of every person’s condition. He understands that he must work efficiently with his peers, both surgeons and psychiatrists, to make sure that patients have the appropriate care. Sometimes, Henry will have to redirect patients to the aforementioned specialists as they are the only figures who can aid them. Furthermore, he admires the complex dynamic of hospital work because there is a sense of teamwork that drives them.

What plans does your character have for the future?
Henry hopes to marry at some point in his 30s. Afterward, he hopes to adopt children of his own and start a family. Of course, he sees himself working and helping patients with their personal health concerns for the rest of his life. A friend of Henry’s who lives in Karakura told him that violence is unfortunately all too common in the town. Due to this, Henry wants to do everything in his power to aid the people who need it. He wants to provide serenity regarding health for the people of Karakura. Professionally, he hopes to be involved in research for skin cancer and attempt to find a cure one day. At the hospital, he hopes to gain more experience with lab research and other clinical practices that can further advance medical assistance for citizens.

How is your character perceived by others? How does it contrast with their perception of themself?
He is described as a social butterfly by the people around him. He likes the concept of being a social butterfly because he already acknowledges that he likes socializing with people and starting friendships. To him, he holds the concept near and dear to his heart because he feels that he is spreading positive energy to the people around him.
Henry is also very sentimental. He keeps items that hold a lot of meaning and symbolism in his life. He still has a letter of recommendation for medical school from one of his past professors, who commended his work in the classroom. He takes a look at it every few weeks because it reminds him that he has the ability to do fantastic work each and every day.
