KARAKURA HOSPITAL APPLICATION

Დ | Out-Of-Character (OOC) Section | Დ

IGN (In-Game Name):

TotallyNotOpxxi [ALT]

Previous bans:
None

Describe your activity on the server?:
In the span of a week, I'm on for a minimum of 5 times, if it's a relatively relaxing week then I'm on every single day for the majority of the day. This goes for my Main account and if this application gets accepted then it’ll be going for my Alt as well because it will be giving me something else to do SRP wise. So, I hope you’ll give me that chance.

Which timezone are you in?:
ADT/AST

Do you have discord? If so what is your tag?:
Ashy<3#9608

Do you have a microphone?:
Yep, and I’ll use it when necessary.

List your current and past applications:

What experiences do you have with Detail RP?:

I’d say that I'm relatively skilled at DetailRP. I’m naturally a quick typer which is why I’m able to make my backstories so detailed and long, but also because I can DetailRP quickly. Whenever I play on SRP, it just improves how well I can DetailRP because I'm reading and seeing what other words people put, and what other adjectives would fit my style of roleplay, therefore I am improving every single day that I’m online and running around aimlessly. Also, I’ve done a lot of GangRP and I’m looking at getting away from it. It was fun whilst it lasted but after around a full year I believe that officially I’ve outgrown it. Besides GangRP, I've done my fair share of JockRP with multiple characters and I'm really just looking for a different type of roleplay to indulge in.

What is your motivation to apply for EMS and how will you benefit our current team?:
My motivation for applying is simple. A while ago I originally wrote an application for a psychiatrist but later on figured out the position was full so I never posted it, although that spot is available now I’ve taken the time to consider that I feel that I want to do something more exciting. Which is why currently I’m applying to be a Surgeon, there’s multiple reasons for why I want to do this but the main one being I honestly just want to meet new people through SRP and the Town Faction as a whole. I already know multiple people that are involved with the faction, but I think that it’ll be a good fit for me. Another reason I would be a good fit is because I'm confident in my communication skills, meaning ICLY I know when to act professionally in front of people, and when it's appropriate to act a little ‘loose’. I have multiple examples of what I mean by that, but I trust you already have an idea of what I am attempting to say.

Which medical role are you applying for? (e.g: Surgeon, Doctor, Psychiatrist):
Surgeon

Specify your knowledge for the role you’re applying for:
A Surgeons job is to diagnose patients, prescribe medication, do pre-op and post-op work, treat the injuried, diseased, the physically deformed and engage in surgical practice under the supervision of their attending, or hospital director.

Categorise all the functions of the EMS and tell us what each of them does:

Director

The director oversees every segment of the hospital ensuring it’s operating as it should, making sure the employee’s are doing their responding jobs correctly and professionally.

Clinical Manager

The Clinical Manager oversees the hospital but focuses more on the individual Clinical Leads. They partake in their own administrative duties as well as helping out around the hospital when necessary.

Clinical Lead

The Clinical Lead(s) observe and train the people in their own field of specialty. They also watch what their lower-position doctors/trainees are doing to make sure they aren't being unprofessional, giving them tips along the way on what to do and not to do.

Surgeon

A surgeon is a type of physician trained to partake and do surgical procedures to treat a long variety of physical illnesses. Some surgeons find a certain specialty to focus on and are better in, while some just stick to having an equal amount of knowledge about each department.

Doctor

A doctor is someone who is trained to deal with physical illness or injury but cannot partake in doing a surgical procedure. They are better at interacting with patients, diagnosing them and then possibly sending them off to a surgeon if they think that their case requires surgical intake. If it can be treated through medicines/creams then they will do that.

Psychiatrist

A psychiatrist is trained to help people that are dealing with Mental-Illnesses/Disorders that can be helped through therapy sessions and/or medications. Psychiatrists treat more in-depth conditions than psychologists.

CHARACTER KNOWLEDGE

Tell us about your character; how do they look, what makes them unique?

Tabata Tsukiko would be a 5’7 Japanese female with gorgeous shimmery blue eyes and long straight dark brown hair which drifted just above the waistline of her pants. She’s overall a quiet person but really brightens up once you get to know her, most people see her as that one ‘overly kind’ person, but really there is no way to be overly kind to anyone. The few people that managed to see her exhausted side or flat out fed up side can say with confidence that she is most definitely a unique human being, and scary when she wants to be.

What are they like on and off the job?
On The Job
: Whilst participating in her own workload of surgical and/or doctor duties, she’d act professional in front of both her own patients and co-workers. Tabata is on the less talkative side of things but once you get to know her personality you might even find it annoying depending on how much she talks. Apart from that, she’s a well rounded-likeable person for anyone to talk to or get to know.

Off The Job: She is not much different when she is off-duty, Tabata is naturally a professional quieter person. Participating in the various events that come into the small town of Karakura and trying to gain a small pool of friends and maybe a possible love interest when it comes to that point.

Outlook on their co-workers? Plans for the future?
Tabata has a high outlook on her co-workers and just peers in general. She respects them even if they are at a lower point then she is in their career or mental health. Trying to help out in any way she can. It’s something that she’s kept since childhood, to not judge anybody based on what they wear, their race, their religion. . etc. Especially her teachers/superiors who have taught her along the way. Not just in her surgical skill but even back to when she was in high-school. Always wanting to have a mutual sense of everyone around her, hoping others will appreciate the hard work she puts in.

Does your character have any past / current mental and or physical illnesses we should take acknowledgment to?
Tabata has a mild case of Sensorineural which requires her to wear a hearing aid, it doesn’t affect her surgical skill or ability to work as the case she possesses isn’t very serious.

BACKSTORY

Let’s turn back time, back to the year 1986 on the first of April in Tokyo, Japan Miss Tabata Tsukiko came into this world with an extravagantly loud cry that I assume was heard throughout the delivery floor. Both of her soon to be married parents Akamine Tsukiko and Koike Kurou were just overjoyed to see their little girl come into this world with a full head of dark brown hair and crystal blue eyes as they shimmered on the lights around the room. Let’s just say, Tabata most definitely did not grow up poor, if anything some would say she was a tad bit spoiled. Always having the expensive toys that other children wanted to steal and get pouty over, nonetheless you’d think that Tabata would have a ****y-overdramatic attitude but she was actually more on the quiet side of things, quiet and kind. Her childhood appeared rather normal, with loving parents who were quite busy but always stood by her. If no-babysitter was available or up to her parents standards she simply accompanied her parents to work and was dropped off at the daycare. Although Tabata preferred to be at home besides at the daycare, she never made a fuss about the decisions her parents made.

Once her 5th birthday approached, her parents decided it was time to move on from staying at the daycare and actually begin attending school, more specifically kindergarten. Both her mother and her father were very over-protective indecisive business associates so simply sending their ‘Darling Daughter’ to any old school wouldn’t do for them. It took them a whole 2 months just to choose a school for their little girl to attend kindergarten, and they did choose one. A private all-girls school that held grades from Kindergarten up till Graduation. Needless to say, at first Tabata really didn’t enjoy going to school. She didn’t like the fact she had to get up early or see other kids, some things never change as you can see. It took a while but after around 4 months of staying there and getting to know her surroundings better she warmed up to the school, especially some of the teachers as well! Always calling her the golden girl. Well, things weren’t all great. Once she turned 7, in grade 1 Tabata started experiencing some ringing in both of her ears and found it weird but ignored it for most of the part as it was bearable and she didn't want to make a scene. Her parents only noticed this when they kept asking her the same question and she either turned and walked away as she didnt hear it, or she asked them to repeat the question. That’s when her parents took her to a hearing specialist where she was later on diagnosed with Sensorineural, a condition which affects her ability to hear things fully and efficiently. She wasn’t fully deaf, most people just call it ‘Hard Of Hearing’. As she could still hear, just not as well. This problem was solved for the most part when her parents acquired her a hearing aid, Tabata liked that she could hear better with it in, and she fortunately had a less-serious case of Sensorineural. The only thing she didn’t like was that her class-mates were constantly asking questions about what the ‘Thing in her ear’ was. After explaining it for the seventh time in one day, her teacher took the initiative to stand up in front of the class and explain what a Hearing Aid was and how it helped Tabata hear her surroundings better. Her class-mates actually found it really interesting despite only being 7 and 8 year olds, and were respectful about her condition.

It was weird for her, but when her classmates accepted her condition it almost made her want to help people like what that nice doctor did for her. This is where her strive to become a surgeon came from. A simple thought from a 7 year old girl turned into her entire career later on in life. Years went by and besides her issue with hearing, all was normal. Tabata was easily the most gifted student in her grade, and that stayed the same for the 13 years she attended school, from kindergarten all the way up to grade-12. Glossing over the boring details of a high-schooler who studies non-stop. The fact that her grades were so high in such an elite private school attracted her teachers and principal's attention, not in a bad way, but in a good way. At the start of her senior year, Miss Tsukiko is already getting scholarship offers to multiple high-end universities and colleges across the world, including some ivy-league schools. Once the school-year came to an end and she was putting on her royal blue robe to walk across that stage, she had over 100 college offers, 20 of which offering full scholarships. There were so many thoughts racing through her head on that day, some were wondering what she was gonna do for a job but her heart already knew, she wanted to be a surgeon, someone to help people in need when they were at their worst. And of course, her mother and father were in full agreement with her plans to become a surgeon. Which is why, Tabata Tsukiko Graduated from highschool on June 25th 2004 as Valedictorian, with Summa Cum Laude, the highest possible honours award you can achieve around the globe. The look on her family’s face was priceless as their eyes filled with tears as the thought of finally needing to let their little girl go out on her own, and enjoy what this annoying, challenging world has to offer.

There was a high, after she graduated. Tabata partied A LOT for that one week and hell, she earned it because as soon as the week period was over she had to begin looking through these college offers and applications. Sorting them into piles; ‘Ivy-League’ - ‘Scholarships’ - ‘Average’ - ‘Bad Offers (No Thought)’. It took around 3 weeks to fully go through all of her college offers, money was no issue during this so it was all about what school she wanted to attend and which she thought was best. Unlike most high-school graduates, she never had a job, not even a small one because she never had the need for acquiring money that she could simply ask her Dad for. But alas, she did decide to get a small job for the summer just so she could get a feel of what it would be like working. At the end of the summer, deadlines were approaching for her college acceptance letters, and she’d narrowed it down to two schools; Harvard Medical School and Stanford University. She loved Stanford for its inordinately humongous campus, but if you get a scholarship to Harvard, anybody knows that you definitely do not turn it down. It was a big move, it's a big move for anyone moving across the world like that but if it was gonna improve Tabata’s dream to be able to aid people in the way she wanted, then it didn’t matter how scary it was or what the cost was. She was gonna go on and do it anyway, which is why at the end of the summer on August 27th she made her big move, of packing up her stuff and hopping on that non-stop 13 hour flight to California. The plane ride was exhausting, being up in the air for that long is never fun.

As her plane came to an abrupt land, all Kiko wanted to think about was crashing in those mildly uncomfortable Harvard dorm mattresses, but at that point in time she didn't care if they were comfortable or not, she just wanted to sleep. Getting to the dormitory was a whole other problem by itself considering the fact she did not know her way around California, but to her luck she actually managed to find another young gentleman that was attending the exact same program as her. She was exhausted, but nonetheless she shook the dude's hand and later on learned his name was Naito. After she arrived at the dormitory, her and Naito parted ways but stayed friends throughout her time there. Tabata Tsukiko did her 4 years for an undergraduate degree and then went on doing another four years at Stanford to get her Medical Degree. I won't explain the sleepless nights she endured and how exhausting it was for her, if you know what I’m talking about, then you know. Going through all this time, it's almost weird to say she never had a romantic relationship, her mind was always on her career to become a surgeon and not who she would date, and quite frankly, nothing has changed. Once she completed her time at Stanford, she looked for places to do her residency, having lived in the largest cities in the world for her entire life; Tokyo and California - She wanted to get a break from the living in such large places but decided there was a need for her to wait, as the best spots for surgical residencies are in the hospitals with the largest amount of trauma cases, the bigger cities. So, she decided to go back to Tokyo and take her surgical residency at St’ Luke's International Hospital. She was a resident there for 5 years, a whole 5 years. And, she's now looking for a new job as a surgeon, somewhere elsewhere, somewhere smaller away from the big cities. While she was cowering the internet looking for smaller towns in Japan, she came across a not-very well known one but had a good enough population for good jobs to occur, called Karakura. It was surrounded by water with one bridge leading in and out of it with a train station on the opposite end, and, she though to herself "Mise well give it a shot." as she looked online, purchasing her train ticket. Tabata realized moments after she purchased the ticket that she didn't have a place there, she was used to her fancy dorms or the places her parents had set up for her during her residency, but to her luck she did find a deluxe apartment for only 50, 000¥ a month. And before you knew it, Tabata was on a train, heading towards Karakura, with her hopes up high. Putting her earbuds in she'd crank up the music on her phone and gaze out the window, waiting to see what was coming next in her journey as a surgeon.
