There is a difference yes, The family is a family and the clan was a criminal organization owned by an Akihtio (That is from my knowledge)
The difference between them was the Akihito clan was the crime side to it or the crime organisation. Basically they did all the crime stuff and were heavily involved with crime. The Akihito family is the family side to the Akihitos. They are one of the Itsbyoshi families. They’re the basically all blood related and share the same last name.
The akihito clan was an yakuza gang. Run by Mike akihito a blood akihito and at the time mayor. The akihito family is just a family mostly comprised of staff who are leaders of factions. Most of them are blood but some like muffin or byeol akihito married into the family. The family usually does not do crime infact mike died when the family found out about him running the akihito clan. That is what i know and have heard so im sorry if this is not right. But thats the gist of it. Its like how ketsueki was the clan did the crime and the family was just a family except ketsueki isnt a main family in the lore of karakura like akihito.
The once powerful Otori family has been ousted after the infamous Otori Corruption Years (1946-1956) but their criminal clan might still have some old roots in the criminal underground of Karakura. There are some Otori family members still living in Karakura today, but most of them are not faction leaders and to my knowledge, no staff members are part of the Otori family. The Otori family has ties to Martial Arts and some economic connections to China (from what I heard).
