Missing Item Name: Hairbow 'Spectral Bone' (or something similar, I cannot quite remember, it is a black hairbow with a skull in the center), one of sleepys3b's custom items
What happened. . .: I went to open my planner with the hairbow on my head, and it glitched into a planner as well. . . Then, it disappeared fully. :(
Proof: This is a clip given to me. In it, you can see the hairbow on my head, which vanishes after I open the planner.
