What’s your motivation to apply for EMS, how will you benefit our medical team?:
My motivation for applying to EMS is to experience a new type of roleplay. I have always been interested in MedicalRP but never quite was able to join the staff. Recently my character has started to attend therapy at Karakura Hospital. Seeing how the team works together inspired me to finally apply.
I believe I can benefit the team with my activity towards the server and my drive to work. I have done numerous jobs on SchoolRP and bring loads of experience. I currently work as college professor teaching law and a server at LCN. In the past I have worked as a high school history teacher as well as a KPD cadet, an art teacher, and a school receptionist.
I am on discord as well as SRP majority of the day. I would be available for patients during times no one hospital staff is normally on as well.
What medical role are you applying for? (e.g: Doctor, Nurse, Psychiatrist, Surgeon.):
I am applying to become a Doctor.
Specify your knowledge of the role you’re applying for?:
Doctors are one of the most important jobs in society. They are the ones who take care of us when we are sick. They are required to go through years of medical school and years of experience before legally practicing medicine. Their main job is to take care of those who come in and need medical attention. This can range anywhere from a simple check up to dealing with broken bones. A doctor must be ready to properly assess and take care of any issue that comes through their door.
They must have a vast knowledge of different illnesses and disorders to be able to properly help their patient. Majority will spend extra time on their own to research more of what they find during their shifts. They must also be ready to sacrifice multiple nights of sleep or even days without going home for their job. There is never a guarantee of who will walk through the doors, and many must stay overnight for patients. Rooms inside are often available for doctors who must rest from long shifts.
Doctors can also find rare cases known as Zebras. These are patients who come in with a rare or unknown ailment that the doctor must find before it's too late. They are important to be taken care of as soon as possible due to the unknown nature they carry. While every patient is important those with more terminal or mysterious issues will be the first priority as they have more to lose.
Are you aware of the basic color codes for medical emergencies? If so, specify all the ones you’re conscious of with their meanings.
Code Red: Fire
Code Orange: Hazardous Spills
Code Pink: Baby Abduction
Code Brown: Severe Weather
Code Blue: Heart Issues
Code Silver: Active Weapon / Hostage Situation
Code White: Evacuate Hospital
Do you take acknowledgment to the fact that if you’re inactive for a long period of time you will be demoted, and do you respect this fact?
Yes, I acknowledge and accept this fact.
Categorize all the EMS roles and specify what each of them does:
Head Physician
This role is in charge of overseeing the hospital and all those that reside inside of it. It is their job to decide what happens inside the hospital, such as who gets hired and what changes happen. It is the most important job out of all of them.
Clinical Manager
This role is the second in charge over the hospital, focused more upon the staff itself. Their job entails making sure everyone else below them does their job correctly. If there is an issue involving the workers it will most likely go to them. If the Head Physician for some reason is unavailable, these are the next ones in charge.
Clinical Lead
Like the Clinical Manager, the Clinical Lead looks over employees. Except they are focused on a specific department. Their main job is to make sure their designated division does the best that it can. Any issues that transpire within those areas will report to their Clinical Lead. This includes reporting or suspension if necessary.
Surgeon
Surgeons are the highest level a doctor can achieve. They must go through rigorous schooling as well as experience to complete their job to the best of their abilities. Any major injuries that require surgery or otherwise serious operations will be dealt with by them.
Doctor
These staff are in charge of regular patient care. When someone needs a check up, broken bones, or just ill they will go to the doctors. They are required to go through Med School as well as a few years of residency to be able to legally practice. Although they don’t perform major surgery their job is just as important.
Nurse
Nurses are the assistants to doctors and surgeons, tasked with minor duties such as prepping the patient for the doctors or taking care of small cuts. Although they don't do as much they are incredibly important to the hospital as well. They can refill prescriptions if need be and do the more mundane tasks around a hospital.
Psychiatrist
Psychiatrist are those who take care of the mental health of patients. They specialize in different types of therapy as well as different mental disorders or illnesses. Their main job is to talk through their patients darkest demons and direct them on a path to recovery. They are able to prescribe medication if need be and have medical degrees. A psychiatrist's job is just as important as any other hospital worker.
IC (In-Character) Information / knowledge
______________
How does your character act during their shift and not during their shift?:
During his shift Jay would be extremely professional. His first priority would always be his patients, ready to do whatever was needed to help them. You would never see him stop working as he's known to be a workaholic. Around his co-workers, Jay would be sociable. He tries his best to create a peaceful work environment for everyone around him, giving a smile from across the halls. His job always comes first for him and that includes everyone inside the hospital.
Off his shift he can usually be seen with his fiance' and their kids. He cares for his family with all his being, especially his soon-to-be wife. If he is not with his family he is most likely studying. One of Jay's favorite pass time is learning. Nearly all of his room would be stacks of books and new things to learn. If found around Karakura he would give a friendly smile to passerbys, and help those in need wherever he goes.
Describe your character’s personality / personification in summary:
Jay is one of the hardest working people you will ever meet. There is never a moment he is sitting still, whether that's the ADHD or his everyday work ethic is up to you. He's friendly to those he's close to and professional to the rest. He often can be seen as the quiet type due to his rather introverted nature. Most of the time Jay is instead engrossed in one of his favorite books. His drive to learn fuels everything he does in his life. Even if he has his inner demons, learning to help others because of them is part of the reason he's still here.
Describe your character’s appearance:
He would be a half-australian half-german male standing around 6'5. His hair would be light blonde with flecks of brown, paired with green eyes. The male would be quite in shape and muscular for his build. He would have an approachable air around him.
Does your character have any past / current mental and or physical illnesses we should take acknowledgement to?
Jay was diagnosed with Asthma at age 4. He was then diagnosed with ADHD when he was around 7 years old.
During his early 20's he was diagnosed with a mild case of PTSD due to his parent's death. He was also diagnosed with anxiety and depression.
He has been in therapy for these and the past few months has been the best he's been his entire life. It's because of the treatment he received at Karakura that he wishes to join their staff team.
Character Background
Jaykeb Solyven Cooper was born on March 25th, 1984 in South Wales, Australia. From a young age he loved to travel the world and explore. His parents would take him on many adventures to different countries. The young boy with ADHD revelled in all the movement. When he was about 11 years old his younger sister Juliet was born. The two were immediately inseparable, his only sibling. They would spend most of their time together including on their families trips.
When Jay was ready to start highschool he decided to move to Japan. He studied rigorously to be able to become a japanese investigator, in love with learning about law. At the same time he became invested in the medical field. He graduated early, his dual enrollment in the local college helping this achievement. Near immediately he threw himself into his work to be able to attend med school. His dream had shifted to one that could save others.
Tragedy struck near his 20th birthday when he received a call from an unknown number. Both his parents had passed in a house fire while his sister was at school. The only family he ever knew was gone.. Shortly after he was adopted into the Cortez family along with his biological sister Juliet. They both gained two adoptive siblings that tried to help them understand this difficult time in their lives.
But Jay wasn’t the same man anymore. He immediately started to fall into a downward spiral inside. Jay continued his studies, eventually graduating med school and becoming a cadet despite his mental health. He poured himself into his work as a way to forget the pain inside. It never truly went away.
He began to drink after he was let go from the police academy, becoming a front desk assistant at Karakura Highschool and eventually a teacher. After classes he would lose himself in the alcohol to blind the memories. It wasn’t until his kids and sister helped him see a problem that he decided to get help.
Jay was diagnosed with PTSD, Severe Depression, and Severe Anxiety at the age of 27. Soon after he was given medication and a rehab program. It wasn’t easy but he slowly started to get better. While he continued his career in education, even becoming a professor, he never forgot that dream of becoming a doctor.
Years later, he finally fell in love with his now fiance. He realized this was the time to decide what they wanted for their life together. He was no longer satisfied being a college professor. She encouraged him to finish his residency and become the doctor he always dreamed of being. Jay agreed, returning to the hospital and giving it his all.
He finished his residency at the age of 34 and spent the past 2 years working and volunteering to get the experience needed to officially join the Karakura EMS team.
